Yoga and related movement involve physical activity and may cause injury. Risks include slips, falls, strains, sprains, dizziness, fainting, aggravation of an existing condition, contact with other participants or equipment, and—in rare cases—serious injury or death. I understand that these risks may arise from my own actions, the actions of others, the condition or use of equipment or the event space, or the ordinary negligence of the persons and organizations involved in the Event.
I choose to participate voluntarily and accept these known and unknown risks. I will participate within my own abilities, follow reasonable safety instructions, and stop if I feel pain, dizziness, shortness of breath, or other discomfort. I understand that the instructors and Event staff do not provide medical advice, diagnosis, or treatment. I am responsible for deciding whether I am able to participate and for consulting a medical professional if I have questions about my health or ability to participate.
In exchange for being allowed to participate, I release and agree not to sue the Los Angeles County Museum of Natural History Foundation, the County of Los Angeles, Mindful LA Ltd LLC doing business as SoulPlay Yoga, and their respective trustees, officers, employees, instructors, volunteers, and agents (collectively, the “Released Parties”) for claims arising from personal injury, death, or property damage connected with my participation in the Event, including claims caused by a Released Party’s ordinary negligence.
This release does not apply to gross negligence, reckless or intentional misconduct, or any liability that cannot legally be waived. It applies only to the Event and related activities on September 13, 2026.
If I need emergency assistance and cannot consent, I authorize Event personnel to contact emergency services. I understand that I am responsible for costs charged by medical providers or emergency responders.
I have read this form, understand that I am giving up important legal rights, and sign it voluntarily.
